Sea Survival

"Never underestimate the sea".

There are always risks & dangers when we are at sea. Personnel who are used to being at sea are always mentally prepared to deal with all kinds of emergencies that might occur:
- Accident
- Medical emergency
- Damaged ships, machinery & equipment
- Fire
- Bad weather
- Out of fuel

Personnel who travel at sea & work at sea must know what to do when difficult & bad situations occur so that lives can be saved:
- Being calm & don't panic
- Avoiding risks & dangers
- Planning the right action for all parties, so that rash actions taken without thinking can be avoided
- Steering the ship to the right location
- Saving drowning passengers
- Dealing with emergencies on Ship
- Communicating using radio
- Dealing with victims
- Conducting First Aid
- Asking for help from the Rescue Center / Coast Guard

The ability to save oneself & have a mental attitude that is responsible for maintaining life is obtained by taking the right course.

First Aid

First Aid is medical care given to someone who is:
- Experiencing illness or injury (burns, cuts, insect stings)
- Being in the middle of a medical emergency

First Aid is useful for:
- Preventing minor accidents from getting worse
- Saving lives

Procedure for doing First Aid:
- Search for anything that might be dangerous in the location (fire, violence & more)
- If safety is threatened, seek assistance from the appropriate party
- Assess the condition of injured or sick victims
- Help to restore breathing so that the victim can regain consciousness by loosening the clothes around the chest & neck & pressing both hands in the middle of the victim's chest several times
- Provide extra care to victims by placing them in a cool & comfortable place, providing blankets, soothing them, treating injuries & more
- Call for medical assistance if the victim requires emergency medical treatment

Some equipment to be used when doing First Aid:
- Cold water to drink & clean the wound
- Bandage to cover the wound
- Required medication (pain reliever, aspirin & others)
- Antibiotic ointment
- Gloves
- Mask
- Thermometer

Accident, Emergency And First Aid

A note to always keep in mind: Accidents & emergencies can happen suddenly at any time in any place.

Accidents & emergencies cannot be handled properly if the people who are at the scene of the accident & emergencies cannot provide First Aid.

Who should have the ability to provide First Aid? Heads of families, family members, employees, company owners, location owners, shop owners & more. The point is:

“Everyone should have the ability to provide First Aid”

Ability to provide First Aid that everyone should have:
- Reawaken the victim who fainted
- Handle breathing by giving CPR
- Reduce pain
- Treat wounds & bleeding (internal & external)
- Handle injuries
- Minimize the consequences of accidents & emergencies

I am happy to watch the “Looking Ahead: Digital in 2023 and Beyond” Event by Simon Kemp.

Here’s my other mind about the information that Simon has shared. His discussion that steals my mind is about “Wash Trading”. His definition of Wash Trading is:

“Wash Trading occurs when traders buy and sell assets that they already owned – often multiply times – in order to make those assets appear more valuable”.

It directly hit my mind because it is done by startup owners.

Is it good or else? I think it all depends on the purpose of the business that we do. If we want to use and practice the business knowledge we get and have and want to get the benefit that we can get through the business and company we have, it is good to have and own it as long as we can. AI Day 2022 by Tesla

Some of the interesting things I learned from AI Day 2022 by Tesla are:
- Robots are artificial muscles and power
- Humans are the main brain of Robot
- Robots can replace humans to do difficult, heavy, and dangerous jobs

My prediction is that in the future, the world will be filled with:
- 75% of Robots act as “artificial humans that move and work”
- 25% of humans who act as "the real brain that drives the Robot"

Yes, Robots are the Artificial Humans.

Personally, I also learn that by having Robots – Artificial Humans, not all humans are required to make new real humans.

Another interesting thing I learn from AI Day by Tesla 2022 is:
- Tesla is very open to sharing all its detailed information. Nothing is hidden and nothing is secret. All the work they do can also be studied, imitated, and done by other companies. This means that other companies can also work and act as “artificial Tesla”.
- Elon Musk provides a place and opportunity for his team to be the keynote speakers according to their knowledge and abilities. This means that Elon Musk is a good leader, humble, and doesn't have a monopoly on power and popularity.

Working in Confined Spaces can be dangerous & poses a risk of serious injury & death. When working in a Confined Space, a few things are bound to happen are:
- Exposure to harmful substances
- Lack of oxygen
- Water buildup
- Lack of light
- Dust in high concentration
- Hot temperature
- Toxic & flammable gases, fumes & vapors
- Bad weather
- Difficulty in evacuating
- Fire

A few things to think about & manage when working in a Confined Space:
- Permit To Work
- Duration
- Work done & how to do it
- Physical condition & health
- Number of workers & Rescue Teams & their abilities – outside & inside Confined Space
- Training requirements
- Confined Space situations & conditions
- Risks & dangers

Workers & Rescue Teams working in Confined Spaces must have the ability to:
- Work in Confined Space
- Control physical hazards & atmospheric hazards
- Protect themselves, including protecting their breath
- Enter & exit Confined Space
- Take appropriate action when an emergency occurs
- Use equipment in Confined Spaces (Gas Detector, Tripod, Davit, PPE, communication device, Harness, Rope, SCBA & more)

Fact: The injured & killed while working in Confined Spaces are workers & people trying to carry out rescues without using proper equipment & never having proper training.

Therefore, companies that have Confined Spaces must have a competent team to help manage risks in Confined Spaces & ensure that all workers & Rescue Teams are adequately trained & instructed.

CPR (Cardio Pulmonary Resuscitation)

CPR (Cardio Pulmonary Resuscitation) is an effort to restore the ability to breathe & blood circulation in the body. Breathing & blood flow that stops can trigger brain damage that can result in a person dying in a matter of minutes.

When performing CPR:
- Victims must be moved to a safer place
- Contact the nearest medical personnel/hospital
- Check victim's consciousness by asking for name, checking pulse & blood flow
- Shake the victim's body slowly
- If the victim responds, try to keep the victim conscious until medical help arrives
- If the victim does not respond, perform CPR
- Lay the victim's body on a hard & flat surface
- Position yourself on your knees beside the victim's neck & shoulders
- Place one palm of your hand on the center of the victim's chest & place your other palm on top of your hand
- Straighten your elbows & place your shoulders directly over your hands
- Press the victim's chest 100–120 times per minute at a rate of 1–2 pressures per second
- Check if the victim can breathe again & shows a response
- If the victim has not responded, you can start trying to open the victim's airway to give artificial respiration
- Lift the victim's head & place your hand on the forehead
- Lift the victim's chin to open the airway
- Pinch the victim's nose, then place your mouth to the victim's mouth
- Give 2 breaths of air from your mouth while seeing if the victim's chest is raised like someone is breathing
- Repeat the chest compressions 30 times followed by giving 2 breaths

Emergency On Helideck

Some of the accidents that usually occur at Helideck:
- Helicopter crashes into Helideck
- Helicopter crashes into other Helicopters, ships, and some facilities on the Helideck
- Dislodged items are then thrown into the Helicopter
- Helicopter cannot land safely on the Helideck

Some of the above can cause a fire in the Helicopter & Helideck.

There are a few things to do to prevent an emergency in Helideck:
- All facilities & equipment at Helideck must always be maintained & inspected
- Before the Helicopter lands & takes off, the Helideck must always be inspected & in a clean & safe condition
- Officers at Helideck must be able to identify potential loose & falling items
- Helideck must have the appropriate person who can provide Landing Safety Instructions to other designated personnel
- Helideck must have officers who can assist with Helicopter Operations

Emergency On Helicopter

An emergency can occur on a Helicopter when:
- Main & tail rotors, engine exhaust, external antenna locations & more on the Helicopter are damaged
- The weather is getting bad
- Helicopter crashes into other Helicopters, planes, mountains & more

When an emergency occurs, the Helicopter will land on the water & can sink & capsize underwater. Therefore, all pilots, crew & passengers must have the ability to:
- Self-protect by using Safety Equipment & PPE, especially life jackets & EBS
- Self-protect by holding the chair & window or exit door
- Escape from the Helicopter which is on the water or sink & capsize underwater by pushing the window or exit door
- Swim to the surface of the water & seek help, including searching for Liferaft
- Survive in the sea

Things to remember:
- PPE, life jacket & EBS must always be used
- First Aid must be given to victims, especially those who are Hypothermic

Fast Rescue Craft (FRC)

The Fast Rescue Craft (FRC) is used for:
- Rescuing the victims when an emergency occurs at sea
- Helping someone to get on another Ship
- Helping the victims who are in trouble

All equipment carried by the FRC must be properly stored on the boat so as not to interfere with the safety procedures. Some of the equipment to bring are:
- Paddle
- Rope
- Spotlight & flashlight
- Compass
- Anchor
- A tool to give a signal
- Signal whistle
- First Aid Kit in a waterproof case
- Required PPE
- Portable fire-fighting equipment that can extinguish fires, especially in Oil & Gas Installations
- Knife & ax

Apart from bringing the required equipment, the FRC must be driven by a proper Coxswain. The Coxswains must do the following:
- Maintain operational readiness
- Prepare & launch FRC
- Communicate using radio
- Use Compass
- Look for victims
- Rescue victims who drowned at sea (Man Overboard)
- Restore the condition of the victims by doing First Aid

Rescue In Confined Space

When working in Confined Spaces, important things to remember:

“Actions to be taken if workers in Confined Space are injured, unable to save themselves & trapped in Confined Space & must be rescued”

Rescue in Confined Space must be carried out immediately if:
- Lack of oxygen occurs
- The workers fell & broke their bones

Steps to take to assist personnel who have to prepare & carry out rescues in Confined Spaces promptly:
- Know the best course of action for rescue in Confined Space
- Conduct rescue training through different scenarios that provide experience to save the lives of others
- Familiarize themselves with situations that can be encountered in Confined Space
- Have faith that rescue training can help prepare teams for work & rescue coworkers in Confined Spaces
- Get used to using the right equipment, especially the Full Body Harness – a place to place lifting equipment that can move & place workers on a stretcher
- Know about the form & content of the Confined Space to be addressed, so that they can use the right equipment (Tripods, anchors, SCBA & others)

The company must always ensure that the proper Rescue Team is always in Confined Space & ready to respond in an emergency as authorities do not always have the manpower & capability to carry out rescues.

First Aid And CPR

When an emergency occurs, First Aid is the only action needed & must be done before the Medical Team arrives.

When a person faints, the most important actions to take first are:
- Checking the victim's consciousness: Conscious or unconscious?
- Is the victim responsive or unresponsive?
- Checking pulse
- Checking the breath

If the victim is not breathing, the next steps to take are:
- Contacting the Medical Team before performing First Aid
- First Aid can be started by performing Cardiopulmonary Resuscitation (CPR)
- Opening & clearing the airway
- Providing respiratory support
- Performing Chest Compressions to maintain blood circulation: Pressing the center of the chest down hard & fast with both hands & letting the chest rise back naturally between compressions
- Turning the body to the side after the victim is conscious & can breathe again
- Checking the victim's condition after the victim can breathe & the heart beats: Is there any injury, broken bones & more?

High Pressure, High Temperature (HPHT)

High Pressure, High Temperature (HPHT) is the term used to define:
- Wells that have high pressure or high temperature
- Wells that have a pressure greater than 15,000 psi (103MPa)
- Wells whose temperature is greater than 350 degrees F (177 degrees C)
- High pressure wells that require well control equipment to have a rated working pressure of more than 15,000 psi
- A well in which the fluid flows at a temperature greater than 350 degrees F at the surface

In reality, very few wells operate under HPHT simultaneously. Most wells operate at high pressure or high temperature but are still referred to as HPHT Wells.

Some shallow wells can be found operating under high temperature conditions due to:
- Wells drilled close to local geothermal hotspots will operate at abnormally high temperatures
- Wells drilled close to other wells that use steam injection to reduce the viscosity of heavy oil will have a higher reservoir temperature than normal because the injected steam will raise the temperature of the surrounding formations

To avoid poor performance & failure in the HPHT environment:
- The metal used is carefully selected
- Equipment must be pressure tested to ensure the seal can withstand high pressure to prevent leakage
- Fluid density should always be checked
- A cooling system must be installed before pumping the drilling fluid back into the hole
- Water-based sludge must always be adapted to the HPHT environment

Travel By Boat

Some tips so that you can enjoy your trip by Boat safely.

Before you travel by Boat:
- Don't forget to think about your Security & Safety
- Plan the trip well before you leave so that risks & dangers can be avoided, including by telling someone about the trip
- Get information about safe & dangerous areas to visit
- Get information about weather conditions, currents & waves in the sea

When you travel by Boat:
- Bring necessary equipment (PPE, life jacket, cell phone, waterproof bag, lamp & more)
- Obey the regulations that apply to Boats at sea
- Wear a life jacket at all times
- Bring enough items so that the boat is not filled with items that can endanger the safety
- Save people who fall into the sea by bringing them back to the Boat as quickly as possible
- Avoid Hypothermia by keeping yourself from cold temperatures & cold water that sticks to clothes, body & others
- Wear dry clothes
- Check the water level before swimming & diving
- Know the limits for swimming & diving
- Don't swim in the sea alone
- Return the one who swims alone in the sea to the Boat

When an emergency occurs at sea:

- Try not to try to swim to the areas that you don't know
- Stay on the Boat & near the Boat while waiting for help

Incident Controller

“Fact: All employers should always be aware of any emergency – Fire, Gas Leak & others – will always happen in the workplace”.

The fact above is a guideline for companies to always have a Special Team that performs certain roles (Security & Safety Team, Fire Team & others). All these teams should be managed by Incident Controllers.

The Incident Controllers are responsible for:
- Creating an Emergency Response Plan & define strategies to control incidents

- Collaborating with individuals & teams according to the tasks to be done
- Delegating authority to act
- Responding to & managing hazards & risks

“First Fact: The Rescue Process in Confined Space can result in more deaths when 1 rescuer & 1 victim quickly becomes 2 victims if the Rescue Program is carried out inappropriately”.

For workers to work safely in Confined Spaces, the company must make regulations regarding:
- Confine Space Program
- Monitor Program
- Hazard Control Program
- Rescue Program
- Permit To Work
- Entry & Exit Program

“Second Fact: Those who suffer the most deaths in Confined Spaces are the Rescuers who don't get proper training & don't use PPE.”

A few things the Confined Space Rescue Team should do:
- Get proper training
- Obtain other appropriate training (Fall Protection, High Angle Rescue, Special Equipment & more)
- Have a good health condition, including the health of the eyes, ears, nose, lungs & muscles
- Know all details about Confined Space (shape, size & more)

Confined Space

“It must be remembered that conditions in Confined Spaces are always very dangerous & life-threatening”.

Facts about Confined Space:
- Many workers & rescuers were injured & killed while working in Confined Space
- Confined Space contains toxic gases & hazardous substances so First Aid, rescue, evacuation & emergency response activities are very difficult to do
- The air quality is very poor & the amount of oxygen is not sufficient
- Fires & explosions always happen
- There are always disturbances in the form of smoke, noise, flooding, slippery, heat/cold, radiation, short circuit & darkness
- Safety equipment is often unusable
- Communication often can’t be done by workers, officers & Emergency Response Team

Some things to do in Confined Space:
- Hazards, especially in the form of toxic gases & flammable atmospheres, must be identified & evaluated
- Air & oxygen quality must be tested
- Permit to Work & PPE must always be owned & used by workers
- Fires & explosions must be prevented & avoided
- All hazardous energy sources (electric, mechanical, hydraulic, thermal & others) must be removed, controlled & isolated

Rescue Craft

Rescue Craft is the most important part of the Safety Mission, Search & Rescue Operations at sea which can assist to rescue people affected by incidents. Rescue Craft is used to prioritize actions & provide medical assistance.

The reasons why the Rescue Craft is used:
- It is efficient & lightweight
- It can be launched from the beach regardless of the weather
- It can carry all the necessary equipment to help everyone

Hypothermia

Hypothermia is an emergency & dangerous condition that occurs when humans are exposed to cold & very cold temperatures. Hypothermia can also occur when humans feel cold due to rain, sweat & submerged in cold water.

Some other things that cause Hypothermia:
- Wet clothes
- Insufficient heating
- The room is too cold
- Being outdoors for a long time

When exposed to cold temperatures, the human body begins to lose heat more quickly, then the body temperature becomes lower & eventually the energy in the body will be exhausted & lost.

Consequences of too low body temperature & loss of energy:
- Disorders of the brain, so they become confused, can't think & lose their memory
- Disturbances in the body & muscles, so they can't move, can't talk & feel very tired
- Disorders of the skin that causes shaking & the skin becomes red

Some things that can be done to treat Hypothermia:
- Take the victim to a dry & warm place
- Use warm & dry clothes & blankets to warm the chest, neck & head
- CPR
- Medical treatment

Sea Survival

Humans are tropical creatures & to survive outside a tropical environment, humans must use clothing & shelter to prevent body cooling.

Incidents can occur at sea because the victims:
- Can't swim & then drown in the sea
- Exposed to cold weather & water
- Not using PPE & rescue equipment (life jackets, wetsuits, life rafts & more)

Things to remember:
- Cold air & water will irritate the skin, muscles, breathing, nerves & heart (usually called cold shock)
- Breathing that is not done properly & uncontrolled will disrupt blood flow, nervous system & heart
- Disorders of the heart when at sea can lead to stroke & heart attack
- Inability to control breathing resulting in drowning
- Various disturbances in the body will interfere with swimming performance & other actions that are important for survival
- Disturbed body temperature (Hypothermia) can cause death
- Hypothermia can cause confusion, disorientation, amnesia, cardiac arrhythmias, loss of consciousness & death

Simple tips:
- If you don't need to be in the sea, try to keep your body dry & warm
- If you need to jump to the sea, enter the sea slowly

Advanced Fire Fighting

Fires & explosions occurring in all companies & industries cause death, injury & property damage. The fires & explosions occur for many reasons:
- Employees are not aware of the dangers & risks
- Combustible items (dust, wood, chemicals, metals & more)

Fire Warden

Fires that occur in buildings do pose a serious risk to businesses & companies:
- Dead
- Injury
- Property & equipment damage

Therefore, the activities carried out by the Fire Warden every day to prevent fires are the most important things needed by all companies. Business owners are indeed required to have personnel responsible for preventing & dealing with fires & ensuring safety in the workplace. Confined Space Rescue

Situations that usually occur in Confined Spaces:
- After four minutes without oxygen, workers in Confined Space will suffer from Asphyxia resulting in brain damage & death
- Most of the deaths that occur in Confined Spaces are attributed to personnel trying to save others

Confined Space can be dangerous & fatal to humans.

Confined Space Rescue is carried out to rescue & recover victims trapped in Confined Spaces or places accessible only via Confined Spaces (basements, storage silos, storage tanks, sewers & more).

Confined Space rescue categories:
- Self-rescue: Workers who are in Confined Space can get out & save themselves, usually because they had detected the danger or because a Monitor gave the order to leave the room
- Non-Entry Rescue: This action frees workers in Confined Spaces by using ropes, Tripods & other rescue equipment to pull them to safe areas
- Entry Rescue: This action is conducted by the Rescue Team who enters the Confined Space. This team must do the rescue carefully so they can save others & themselves

Commonly used Rescue Equipment:
- PPE (SCBA, head protection & more)
- Lamp
- Seatbelt
- Safety/rescue rope
- Air monitoring device
- Communication tool

Safety And Survival At Sea

Surviving in the middle of the sea is the hardest thing because humans cannot breathe underwater & accidents can always happen when we are in the sea.

Two types of accidents usually occur at sea:
- Accidents that can be handled immediately: The personnel can immediately evacuate using the available safety equipment (Lifeboat, Life Raft & others)
- Uncontrolled accident: The personnel are forced to jump into the sea

The right way to save yourself from an accident at sea:
- Use a Life Jacket
- Transfer yourself to the available Lifeboats & Life Rafts
- If you have to jump into the sea, try to place your feet first to the surface of the water
- If you are alone at the sea, hug your body by touching your armpits & closing your legs so that your body can stay warm
- If there are several people at the sea, then this group of people must help each other by forming a circle & hugging others so that the body can stay warm & the injured victim can be placed in the middle of the circle
- Group swimming can also be done by placing people in a row & pinching the legs of the person in front
- If there's no Life Raft, you have to look for floating objects that can hold your body from sinking (wood & more)
- Use Smoke Signals (during the day), Fire Signals (at night) & Mirror Signals to inform Rescue Teams, Ships, Planes, Helicopters & more

Fire Fighting

Attendant in Confined Space

The Confined Space that is owned by all companies in all industries is a room that must always be monitored because an emergency can always occur in this room. One of the parties overseeing the Confined Space is the Attendant. Some things that must be done by the Attendants in Confined Spaces:
- Assess the condition of the Confined Space to determine if it is safe for workers to enter the Confined Space
- Ensure that workers who will work in Confined Space have Permit To Work
- Warn people & workers who do not have Permit To Work that they must stay away & should not enter into Confined Space
- Report to the right party if someone enters the Confined Space without permission
- Protect workers who work in Confined Space
- Detect the hazards encountered when entering a Confined Space (signs, symptoms & consequences of exposure)
- Always on standby outside the Confined Space & only enter the Confined Space when an emergency occurs
- Call the Rescue Team when an emergency occurs

* This course by TEEX is designed to train Onshore Fire Team members with the knowledge and skill to safely and effectively respond to a fire. The 3-day course will teach about theory, fire suppression equipment, PPE including SCBA, extinguishers (portable, foam, water & others), fire fighting tactics, recovery, and more. The exercise at Samson Tiara Cilegon Training Center to fight Class A, B and C Fires will use hose lines (water & foam), SCBA and fire props (pans with and without obstacles, paint lockers, LPG impinging jets & XMAS tree, leaking flanges, and large diameter storage tank). Floor / Fire Warden (Warden) by Samson Tiara:

Accident Reporting & Investigation

By applicable regulations, the company must take appropriate action (directing & controlling) so that its employees can carry out their duties properly, be responsible for their safety & avoid accidents.

The categories of accidents that must be investigated & reported are those which:
- Causing serious injury & death
- Causing damage to buildings, bridges, towers & more
- Causing fire & explosion
- Occurring due to the presence of harmful substances & toxic gases

When an accident occurs in the company, the company, through its Supervisor, must ensure that:
- Workers have been saved from accidents & received First Aid
- Scene has been secured & activities are restricted until the accident can be resolved
- All accidents & their victims must be reported immediately
- Party in charge, including the Supervisor, must investigate the accident that occurred
- Appropriate actions have been taken to eliminate & reduce the risk of injury

The investigation process should be carried out to:
- Identify any unsafe conditions & procedures
- Find facts & gather objective information
- Take appropriate action so that the same accident does not happen again in the future

To become Supervisors & personnel who can carry out accident investigations, employees must take courses that can educate & train them to know the investigation process.
