Safety: Accident Investigation

Regulations around the world require that all workplace incidents must be investigated to ensure compliance with safety standards & improve workplace safety. Accident investigations can influence policy, industry standards & public perception of safety.

Accident investigation is an important process that aims to:
- Understand why an incident occurred: The root cause of the accident (human error, equipment failure, environmental factors, negligence)
- Develop & implement appropriate corrective actions, processes & procedures to prevent recurrence

Steps in accident investigation:
- Immediate response: Ensure safety at the scene, provide medical attention if necessary & secure the area to preserve evidence
- Data collection: Collect all relevant information, physical evidence, witness statements, photographs & any available data
- Process for analysis: Reconstruct the sequence of events, analyze root causes & formulate conclusions about why the incident occurred
- Recommendations: Suggest changes & improvements to safety systems, procedures, equipment & protocols to reduce future risks

As an accident can occur at any time, create a safe & productive work environment & improve organizational processes. At the end, proper investigations can be carried out appropriately!

Offshore Safety & Emergency Procedures must be done by all offshore industries, such as Oil & Gas, Energy & others.

Some steps aim to protect humans, lives, the environment & the industry:

- Offshore Safety is governed by strict regulations on offshore, transportation, aviation & others that ensure that facilities operate to high safety standards to reduce the risks inherent in offshore work
- Offshore Emergency Procedures are plans that guide safe actions in the event of an emergency: Handle, rescue, evacuation & others
- Offshore Workers must be equipped with special PPE & Safety Equipment that are tailored to the risks & hazards in the offshore environment
- Offshore Workers must undergo extensive training (BOSIET) that prepares them to handle emergencies, extinguish fires, evacuate & survive at sea
- Safety must be carried out by carrying out work safely, detecting gas, preventing fires, having a Permit To Work in high-risk works & others
- Special equipment with its sophisticated & modern technology is used to communicate & assist rescue operations in real-time: Radio, GPS & others

Security: Workplace Harassment

Workplace harassment is a serious issue that can take many forms:
- Verbal harassment: Bad comments about a person’s race, gender, religion, sexual orientation & character
- Physical harassment: Unwanted physical contact, such as touching, blocking someone’s way & any form of physical intimidation
- Sexual harassment: Inappropriate sexual comments, requests for sexual favors & other behavior of a sexual nature that creates an intimidating work environment
- Cyber harassment: Harassment through social media, such as sending threatening emails & posting bad comments about someone
- Psychological harassment: Intimidating, belittling & humiliating another person

Steps to deal with harassment:
- Record everything about the harassment: Occurrence, time, place, perpetrator, victim & witnesses
- Report it to HR & a person in charge of reporting harassment
- Get support by talking to trusted parties
- Get legal protection

All countries have laws against workplace harassment & require employers to provide a safe work environment. Therefore, workplaces should foster mutual respect by providing clear policies, a culture where harassment is not tolerated & courses.

Important Notes:
- Ignoring harassment can cause further psychological damage & make the situation worse
- All victims have the right to be protected & supported

In Construction, Scaffolding refers to a temporary structure used to support workers & materials during the construction, repair, or cleaning of buildings & other structures.

Scaffolding emergencies occur when:
- Scaffolding collapses due to improper built
- Scaffolding overloading
- Workers fall from a height
- Equipment falls
- Bad weather: Strong winds, rain, storms & others

Some Safety Procedures to be done when using Scaffolding:
- Make sure the Scaffolding is built by competent personnel who follow safety standards
- Make sure the Scaffolding can be used safely
- Workers using Scaffolding must be trained first so that they can use the Scaffolding safely, work properly, maintain safety & handle emergencies
- Workers using Scaffolding must know that there is always a danger when using Scaffolding
- Scaffolding must be checked & maintained regularly to ensure that the Scaffolding remains safe to use
- Use the right equipment: Guardrails, fall arrest devices, safety belts & more
- Use the right load capacity
- Secure the Scaffolding from wind or other environmental factors
- Secure tools & materials when not in use
- Stop work on the Scaffolding if bad weather occurs
- Have Safety Procedures that are tailored to the Scaffolding
- The Scaffolding used must be following applicable regulations
- If an emergency occurs: Provide first aid immediately & contact emergency services if necessary for rescue operations & medical assistance

Safety: Fire Safety Awareness

Fire Safety Awareness is needed to protect everyone & every place: you, your co-workers, your family, your home, your office, your building & more!

Safety is Number 1!

Some things to do to prevent & handle fires:

Before a fire occurs:
- Don't smoke!!!
- Plan special actions for Seniors & People with Disabilities who may require special assistance to save themselves
- Ensure all electrical equipment is used properly, not overloaded & in good condition
- Do not leave machines running
- Turn off machines when leaving the room
- Unplug electrical cables when not in use
- Keep an eye on the kitchen at all times
- Keep flammable items away from heat sources
- Install smoke alarms & fire safety equipment in every room
- Know the fire exits in every place & room
- Know the place to gather after the evacuation process
- Train yourself, co-workers & family to extinguish fires

If a fire occurs:
- Avoid all hot objects & areas
- Put out the fire if you can use a Fire Extinguisher & extinguish the fire
- If the fire burns your clothes, drop your body & roll so the fire can be extinguished
- Evacuate immediately
- Do not enter the burning area until the Fire Team says the area is safe
